What is Orforglipron?
Orforglipron is a once-daily GLP-1 pill from Eli Lilly, sold as Foundayo. Strictly speaking it isn't a peptide. It's a small molecule that acts on the same GLP-1 receptor as semaglutide. We cover it because it's one of the most-asked-about weight-loss drugs of 2026.
Is Orforglipron FDA-approved?
FDA-approved on April 1, 2026, for weight reduction and long-term maintenance in adults with obesity, or with overweight plus at least one weight-related condition (FDA announcement). Prescription only. The label says not to combine it with another GLP-1 drug.
Foundayo dose schedule: what the FDA label says
Your prescriber sets your dose. This is a summary of the official FDA label to help you follow along, not a plan to start, change, or stop anything.
Foundayo (tablet) — FDA label on DailyMed
- Once daily, with or without food. Swallow whole. Don't break, crush, or chew. Never more than one tablet a day.
- Starts at 0.8 mg daily. After at least 30 days, 2.5 mg. After at least 30 more days, 5.5 mg.
- It can go up to 9 mg, 14.5 mg, or 17.2 mg, each step after at least 30 days, based on response and tolerability. Maximum 17.2 mg daily.
- Missed dose: take it as soon as possible. Don't double up. If 7 or more doses in a row are missed, the label says to restart at a lower dose.
- Some other medicines (strong CYP3A4 inhibitors or inducers) change the dosing. Tell your prescriber everything you take.
What the research shows
- ATTAIN-1 (NEJM, 2025): 3,127 adults with obesity, no diabetes, 72 weeks. Average weight change was −7.5% to −11.2% depending on dose, vs −2.1% on placebo. 5.3–10.3% stopped because of side effects (vs 2.7% on placebo). GI effects were most common.
- The trial dose levels (6, 12, 36 mg) don't match the tablet strengths on the U.S. label. Go by the label and your prescriber, not trial numbers.
