What is Semaglutide?
Semaglutide is a lab-made version of the gut hormone GLP-1. It's sold under brand names including Ozempic and Rybelsus (type 2 diabetes) and Wegovy (weight management).
Is Semaglutide FDA-approved?
FDA-approved prescription medicine. Ozempic (injection) and Rybelsus are labeled for blood-sugar control in adults with type 2 diabetes, and Ozempic also for lowering heart-event risk in some of those adults. Wegovy is labeled for long-term weight management in certain adults and children 12+, and for lowering major heart-event risk in adults with heart disease and overweight or obesity. Sources: Drugs@FDA Ozempic, Rybelsus, Wegovy. Exact indications are in the current FDA label.
Semaglutide dose schedule & timing: what the FDA label says
Your prescriber sets your dose. This is a summary of the official FDA label to help you follow along, not a plan to start, change, or stop anything.
Ozempic (injection) — FDA label on DailyMed
- Once weekly, on the same day each week, any time of day, with or without meals.
- Starts at 0.25 mg weekly for 4 weeks (a starter dose, not meant to control blood sugar), then 0.5 mg weekly.
- If more control is needed, it can go to 1 mg, then 2 mg weekly, each after at least 4 weeks on the current dose.
- Missed dose: take it within 5 days. If more than 5 days have passed, skip it and resume on your usual day.
Wegovy (injection or tablet) — FDA label on DailyMed
- Injection: once weekly, same day each week. The label's escalation runs 0.25 mg (weeks 1–4) → 0.5 mg (5–8) → 1 mg (9–12) → 1.7 mg (13–16). The usual recommended maintenance dose is 2.4 mg weekly, and the label gives options by indication.
- Tablet: once daily in the morning on an empty stomach, with up to 4 oz of water. Wait at least 30 minutes before eating, drinking, or taking other oral medicines. Starts at 1.5 mg daily and steps up every 30 days, per the label, to a maintenance dose of 25 mg daily.
- If a dose step isn't tolerated, the label lets the prescriber delay the next increase by 4 weeks.
Rybelsus (tablet) — FDA label on DailyMed
- Once daily in the morning on an empty stomach, with up to 4 oz of plain water. Swallow whole.
- Wait at least 30 minutes before eating, drinking, or taking other oral medicines.
- 3 mg daily for 30 days (a starter dose), then 7 mg daily. It can go to 14 mg daily if more control is needed.
- Rybelsus and Ozempic tablets are not interchangeable mg-for-mg.
What the research shows
- STEP 1 (NEJM, 2021, PMID 33567185): an RCT in adults with overweight or obesity. Over 68 weeks, weight changed by −15.3 kg with semaglutide vs. −2.6 kg with placebo, alongside lifestyle intervention. Nausea and diarrhea were the most common side effects.
- SELECT (NEJM, 2023, PMID 37952131): 17,604 people with heart disease and overweight/obesity but no diabetes. Major cardiovascular events occurred in 6.5% on semaglutide vs. 8.0% on placebo.
- Kidney disease trial (Nature Medicine, 2026, PMID 42823485): a 106-person RCT exploring how it may protect the kidneys in type 2 diabetes. Covered in Issue #1.
